Job Description:
Key Responsibilities:
- Diagnose and treat a wide range of health conditions common in older adults, including chronic illnesses, cognitive disorders, mobility issues, and age-related health concerns.
- Conduct thorough consultations with older patients in person and via telemedicine, providing individualized care plans focused on enhancing quality of life.
- Prescribe appropriate treatments, including medications, physical therapy, lifestyle adjustments, and preventative care to support long-term wellness.
- Collaborate with a multidisciplinary team, including physical therapists, mental health professionals, and social workers, to deliver holistic and coordinated care.
- Participate in family consultations, offering guidance on caregiving, palliative care, and resources available for older adults.
- Engage in case discussions, peer consultations, and contribute to quality improvement initiatives within the hospital and clinic.
- Maintain up-to-date and accurate patient records using our Health Management Information System (HMIS).
- Engage in continuous professional development and contribute to the growth of geriatric services at Fitwell Hub.
