Important Information
General Sickness
Coverage starts 15 days after enrollment.
C-Section (Family Plan
Coverage starts after 6 months of enrollment.
Accidental Hospitalization
Covered immediately.
Required Claim Documents
To process your claim, the following documents are required:
- Claimant statement signed by the insured person.
- Hospitalization records with admission and discharge dates, diagnosis, and treatment details.
- Physician’s statement or hospital bills.
- Copy of insured person’s CNIC.
- Any additional documents requested by Fitwell HUB or the underwriter.
Exclusions
The following are not covered under the plans:
- Intentional self-inflicted injuries, suicide attempts, or murder.
- Pregnancy and complications during the first 9 months from the enrollment date.
- Elective treatments or failure to follow medical advice.
- Pre-existing conditions.
- Drug or alcohol abuse and poisoning.
