Job Description:
Key Responsibilities:
- Provide comprehensive pain and symptom management for patients with chronic and life-limiting conditions, focusing on improving quality of life.
- Conduct in-depth patient assessments and consultations, both physically and through telemedicine, to create tailored palliative care plans.
- Prescribe appropriate medications, therapies, and non-pharmacological interventions to manage pain, symptoms, and psychological distress.
- Collaborate closely with multidisciplinary teams, including primary care providers, oncologists, and mental health professionals, to ensure holistic patient care.
- Offer counseling and emotional support to patients and families, helping them navigate complex medical and end-of-life care decisions.
- Participate in case discussions, peer consultations, and contribute to quality improvement initiatives within the clinic and hospital.
- Maintain accurate and up-to-date patient records using our Health Management Information System (HMIS).
- Engage in ongoing professional development and contribute to the growth of palliative care services at Fitwell Hub.
